Myxedema coma is a rare but life-threatening state of extreme hypothyroidism. It may occur in those with established hypothyroidism when they develop an acute illness. Myxedema coma can be the first presentation of hypothyroidism. People with myxedema coma typically have a low body temperature without shivering, confusion, a slow heart rate and reduced breathing effort. There may be physical signs suggestive of hypothyroidism, such as skin changes or enlargement of the tongue.
=== Gut microbiota === The effect of the low-FODMAP diet on the gut microbiota is not fully understood. It is thought that reduction of fermentable carbohydrates affects the composition and abundance of gut bacteria. FODMAPs are a main food source (prebiotic) for many gut bacteria. Deprived of this food source, there is less bacterial fermentation in the gut and less production of intestinal gas, which may also create conditions which favor certain species of bacteria and disfavor others. There is some evidence for negative effects of the low-FODMAP diet, such as reduction in the numbers of beneficial bacteria (e.g., Bifidobacteria). Such changes are comparable to dysbiosis. Other studies report no significant change in gut microbiota from the low-FODMAP diet. There is also some evidence for positive effects on the gut microbiota, such as improved microbial diversity and increased numbers of potentially beneficial bacterial species. The effect of the low-FODMAP diet on gut microbiota also seems to depend on the medical condition, with more profound changes in microbiota occurring in celiac disease or inflammatory bowel disease, but no significant microbiota changes occurring in IBS. Overall, the low-FODMAP diet may have a positive effect on the gut microbiota compared to normal diets. However, the evidence is mixed and there is significant study heterogeneity, probably because of variation in the methodology and length of the studies, and also differences in the studied populations such as genetics and baseline diet.

=== Modulation === A neurotransmitter may have an excitatory, inhibitory or modulatory effect on the target cell. The effect is determined by the receptors the neurotransmitter interacts with at the post-synaptic membrane. Neurotransmitters influence trans-membrane ion flow either to increase (excitatory) or to decrease (inhibitory) the probability that the cell with which it comes in contact will produce an action potential. Synapses containing receptors with excitatory effects are called Type I synapses, while Type II synapses contain receptors with inhibitory effects. Thus, despite the wide variety of synapses, they all convey only these two types of messages. The two types have different appearances and primarily act on different parts of neurons. Receptors with modulatory effects are spread throughout all synaptic membranes and binding of neurotransmitters sets in motion signaling cascades that help the cell regulate its function. Binding of neurotransmitters to receptors with modulatory effects can have many results. For example, it may increase or decrease in sensitivity to future stimuli by recruiting more or fewer receptors to the synaptic membrane. Type I (excitatory) synapses are typically located on the shafts or the spines of dendrites, whereas type II (inhibitory) synapses are typically located on the cell body. In addition, Type I synapses have round synaptic vesicles, whereas the vesicles of type II synapses are flattened.

A DNA sequence is called a "sense" sequence if it is the same as that of a messenger RNA copy that is translated into protein. The sequence on the opposite strand is called the "antisense" sequence. Both sense and antisense sequences can exist on different parts of the same strand of DNA (i.e. both strands can contain both sense and antisense sequences). In both prokaryotes and eukaryotes, antisense RNA sequences are produced, but the functions of these RNAs are not entirely clear. A few DNA sequences in prokaryotes and eukaryotes, and more in plasmids and viruses, blur the distinction between sense and antisense strands by having overlapping genes. Yearly thyroid function tests are recommended in people with Down syndrome, as they are at higher risk of thyroid disease. Guidelines for England and Wales from the National Institute for Health and Care Excellence (NICE) recommend testing for thyroid disease in people with type 1 diabetes and new-onset atrial fibrillation, and suggests testing in those with depression or unexplained anxiety (all ages), in children with abnormal growth, or unexplained change in behavior or school performance. NICE also recommends screening for celiac disease in people with a diagnosis of autoimmune thyroid disease.
== Re-activity (1980–2000) == In 1980, old friend (and fellow ex-Sinner) Dave Machan persuaded Ron to perform again, along with vocalist Kathy Monaco this time in a folk based group called Trinity (Ron played bass for this group). Trinity recorded an albums' worth of material at Dayson Studios (owned by George Day and also home to much of the work done by Ron Wray for the History of Syracuse Music releases) which still remains unreleased. Though Trinity was successful on the nightclub scene, Ron wanted to return to the rock music that was his first love, and tried to re-form The Saint and the Sinners which soon changed their name to East Side Story (later East Side). East Side specialized in classic oldies and seventies hard rock. This band lasted (with virtually the same line-up) for fifteen years before folding in 2000. Some of the members of East Side were Graham Daniels (1984–2000), Larry Brennan (1984–2000), Tom DiRenzo (1988–2000), John Mirra (1991–1993; 1997–2000), Earl "the Pearl" Hamilton (formerly of The New York Flyers now with Redline (1994–1997), Dave Machan (1984–1987) and Barry Glisker (1984–1989).
=== Medicaid overbilling === A fraud investigation by the United States Department of Justice began in 2000 when allegations were brought in two separate lawsuits filed by whistleblowers under the False Claims Act. They alleged that Merck failed to pay proper rebates to Medicaid and other health care programs and paid illegal remuneration to health care providers. On February 7, 2008, Merck agreed to pay more than $650 million to settle charges that it routinely overbilled Medicaid for its most popular medicines. The settlement was one of the largest pharmaceutical settlements in history. The federal government received more than $360 million, plus 49 states and Washington, DC, received over $290 million. One whistleblower received a $68 million reward. Merck made the settlement without an admission of liability or wrongdoing.
== Function == The cell bodies produce the peptide hormone vasopressin, which is also known as anti-diuretic hormone (ADH), and the peptide hormone oxytocin. Both of these peptides are released from the posterior pituitary. ADH travels via the bloodstream to its target cells in the papillary ducts in the kidneys, enhancing water reabsorption. Oxytocin travels via the bloodstream to act at the mammary glands and the uterus. In the cell bodies, the hormones are packaged in large, membrane-bound vesicles that are transported down the axons to the nerve endings. The secretory granules are also stored in packets along the axon called Herring bodies. Similar magnocellular neurons are also found in the paraventricular nucleus.
